Our Team
Insurance requirements can vary by plan. Some Blue Cross Blue Shield plans, Medicaid, and TRICARE Prime require a referral from your primary care physician before seeing a specialist. Patients with UnitedHealthcare Medicare Advantage and Cigna HealthSpring plans may be required to have a referral and may only be eligible to see Dr. McLeod. Some Blue Cross Blue Shield and VIVA Health plans are limited to specific provider networks, which may not include our physicians.Because insurance coverage and provider networks vary by plan, we recommend contacting your insurance company directly to confirm your benefits and obtain a list of participating providers.
Payment is expected at the time services are rendered. Co-pays, deductibles, and balances are due at check-in. As a courtesy we will file your insurance claim, but the responsibility of insurance verification for office visits, procedures, and surgeries lies with the patient. As a courtesy we will make every effort to provide you with a good faith estimate of your portion.We accept MasterCard, Visa, and Discover. Financing is available with approved credit through Care Credit (GE Moneybank).

Please be sure to bring all your insurance cards, any copays due, and an updated list of medications. If you are coming for CPAP check-up, bring your CPAP machine and power cord. For established patients, we offer same-day appointments for urgent sinus and allergy problems if you call before 9am.
As of March 1, 2020, there will be a charge of $30.00 - $50.00 to fill out FMLA and Disability forms, and can take up to 10 business days. The $30.00 - $50.00 will be due when the forms are dropped off.
NEXUS letter is $200.00 and can take up to 14 business days.
Accommodation letters $50.00 and can take up to 10 business days.
FAA Letters are $50.00 and can take up to 10 business days.
